Since the Disk Tools disk will not revive it, you could have a serious hard
disk problem. However, you should call Apple Tech support first
1-800-800-APPL. This is the education number and is faster to get through
on than the 1-800-SOS-APPL number. They may have some good suggestions, or
it may be a known problem which they will take care of.

You might try opening up the mac and reseating the scsi cords to the hard drive.
