The term does back to the time of Cicero, if not before. From the early
Middle Ages on it was used in the sense described by Joe Ward, in
contrast to the walled area (which was often defined as the civitas). My
1976 paper on 'Suburban growth' (reprinted in Holt and Rosser (ed.), The
Medieval Town: a reader in English Urban History (London and NY, 1990))
goes into some of these issues
